Attention Wellcare Superior HealthPlan Dual Align (HMO D-SNP) Members Receiving Medically Necessary Incontinence Supplies

Wellcare Superior HealthPlan Dual Align (HMO D-SNP) is excited to partner with TenderHeart Health Outcomes. TenderHeart is a leader in providing high-quality incontinence supplies and excellent customer service, at no cost to Wellcare By Superior HealthPlan members.

Why TenderHeart?

  • TenderHeart products are gentle on your skin and absorb well to stop leaks.
  • TenderHeart provides you with your own incontinence coach. They will help you get the right products to meet your needs.

What Happens Next?

  • You have a choice to receive your incontinence supplies from TenderHeart.
  • If you want to keep your current Durable Medical Equipment (DME) incontinence supply provider, please call Wellcare By Superior HealthPlan at 1-833-703-8334 (TTY: 711) and let us know. You must call us if you want to keep receiving your incontinence supplies from your current DME incontinence supply provider.
  • If you do not call us to keep your current DME incontinence supply provider, you will receive your incontinence supplies from TenderHeart beginning October 1, 2026.

o   A TenderHeart coach will call you to make sure you get the right products to meet your needs.

o   If TenderHeart cannot reach you, you will get the same style and size product you are currently receiving.

If you have any questions, please call Wellcare By Superior HealthPlan Member Services at 1-855-445-3556 (TTY: 711). Our team is available. From Oct. 1 to March 31, you can call us seven days a week from 8 a.m. to 8 p.m. From April 1 to Sept. 30, you can call us Monday through Friday from 8 a.m. to 8 p.m.

As a Wellcare By Superior HealthPlan member, you have coverage for both Medicare and Medicaid. You will receive your Medicare benefits and your STAR+PLUS Medicaid benefits through Wellcare Superior HealthPlan Dual Align (HMO D-SNP).
